What is Integrated Community Health Partners (ICHP)?

ICHP is the Regional Care Collaborative Organization for Region 4. Region 4 serves ICHP is a partnership between ValueOptions®, Inc., and several Colorado-based health care/ mental health organizations.

  • Colorado Community Managed Care Network (CCMCN) and its primary care provider organizations, serve Medicaid member’s health care needs.
  • Four Community Mental Health Centers serving ICHP counties.  
  • ValueOptions®, the nation’s largest independent behavioral health and Wellness Company. Collectively, CCMCN, the four mental health centers and ValueOptions have served Medicaid Members for over 30 years.

If you have Medicaid and have enrolled in the ACC program and live in one of the counties ICHP serves, you are a member of Integrated Community Health Partners.

We have created this website to help you get the most from your health care benefits. This web site has tools, information and links to help people get and stay well.

How can this web site help my family or me?

We have put together  tools and information to help you get the most out of your Medicaid health care benefits. All of the ICHP printed materials are here. We also have on-line tools that can help you and your family stay well.

  • Achieve Solutions® This online library offers more than 3,000 articles on more than 200 health topics that cover health and wellness, family care, depression, anxiety, heart disease, diabetes, and financial and legal concerns. Achieve Solutions® also has news articles, videos, audio clips and self screening tools. Anyone can view these articles. Watch a demo on how to use Achieve Solutions®.
  • Community Resource Directory lists free or low cost resources such as childcare, help with utility bills, transportation, school resources and other programs that can help you and your family. The directory uses a search engine to help you find specific resources close to home. All you have to do is type in a zip code and check a category. The directory searches resources for you.
  • Left Hand Navigation Panel. On the left hand of each web page, you will see links that will take you right to our most helpful pages, such as FAQ’s, patient rights, and letters from the state.
