Who is Integrated Community Health Partners?
In 2010, The State of Colorado created a new health care program called the Accountable Care Collaborative (ACC) for Medicaid members. It is a program that will help Medicaid members and their families get the right care at the right time. Medicaid members who belong to the ACC will belong to an “accountable care organization” that will help them manage all of their health care needs.
The ACC is made up of seven regions in Colorado. Each of these regions has a Regional Care Collaborative Organization (RCCO). The RCCO will manage the health care needs of all ACC members who live in the region. ICHP is the RCCO for Region 4.
ICHP is a partnership between ValueOptions®, Inc., and five Colorado-based service delivery organizations including Colorado Community Managed Care Network (CCMCN) and its primary care provider organizations, and four community mental health Centers serving ICHP counties.
